Symptoms, Causes, and Treatments

What is Chronic low back pain?

Chronic low back pain is pain in the lower back region that lasts longer than 12 weeks, even after the initial injury or strain has healed. Unlike acute pain, which serves as a warning of injury, chronic low back pain often continues without ongoing tissue damage. It may fluctuate in intensity and can interfere with work, sleep, and daily activities.

Common Symptoms Include

  • Persistent aching, stiffness, or soreness in the lower back

  • Pain that worsens with prolonged sitting, standing, or bending

  • Episodes of sharp or stabbing pain

  • Muscle tightness or spasms

  • Pain radiating to the hips, buttocks, or legs (sciatica-like symptoms)

  • Fatigue and reduced mobility

  • Anxiety or low mood linked to chronic discomfort

What causes it?

Chronic low back pain can develop from multiple overlapping factors:

  • Muscle or ligament strain that doesn’t fully resolve

  • Degenerative changes (arthritis, disc wear and tear)

  • Nerve irritation (sciatica, spinal stenosis)

  • Postural strain or sedentary lifestyle

  • Stress and emotional tension, which tighten muscles and amplify pain signals

  • Central sensitization, where the brain amplifies normal pain input

In many cases, the pain persists even after tissues have healed, pointing to changes in nervous system processing.

Possible contributors include:

  • Past injury or repeated strain

  • Poor posture or ergonomics

  • Sedentary lifestyle and deconditioning

  • Stress, anxiety, or unresolved trauma

  • Central sensitization (nervous system hypersensitivity)

  • Inadequate sleep or recovery

  • Fear of movement (“pain avoidance cycle”)

A Neuroplasticity-Based Approach

Chronic low back pain is often more about the brain’s interpretation of signals than ongoing injury. A neuroplasticity-based approach helps retrain the nervous system to reduce pain sensitivity, break the cycle of fear and tension, and restore movement confidence.

At Menda, we use therapies that help retrain the brain and calm the nervous system. Approaches such as Pain Reprocessing Therapy, Cognitive Behavioral Therapy, and Acceptance and Commitment Therapy can help reduce the brain’s fatigue signals and restore balance. By teaching the nervous system new, safer patterns, people can gradually rebuild their energy, resilience, and quality of life.

You don’t have to keep living with these symptoms. Explore Menda’s evidence-based therapies  designed to rewire the brain and restore balance.

Key Neuroplasticity-Informed Strategies

  • Somatic tracking: Noticing sensations without fear or judgment

  • Gentle, graded movement: Restoring mobility step by step to reduce fear of activity

  • Mind-body relaxation practices: Breathing, meditation, yoga, or progressive muscle relaxation

  • Pain reprocessing techniques: Shifting the brain’s interpretation of back pain from “danger” to “safety”

  • Stress and tension release: Addressing emotional contributors that keep muscles tight

  • Visualization and positive reinforcement: Training the brain to expect comfort and mobility

  • Improving posture and ergonomics without fear-based rigidity

Why Neuroplasticity Works

With chronic low back pain, the nervous system often becomes hyper-alert, keeping muscles tense and amplifying pain signals. Neuroplasticity techniques calm the brain, reduce threat perception, and allow the body to move more freely. Over time, the brain rewires to interpret sensations more accurately, decreasing pain and restoring confidence in movement.

When to Seek Care

You should seek medical care if:

  • Pain is severe, persistent, or worsening

  • Pain radiates into the legs with numbness, weakness, or tingling

  • You experience loss of bladder or bowel control (urgent medical concern)

  • The pain interferes significantly with daily life, work, or sleep

  • You feel overwhelmed, anxious, or depressed by your symptoms

A healthcare provider can help rule out serious conditions and guide a plan that may combine medical treatment, physical therapy, and neuroplasticity-based retraining.
